2.   Attach the following:

     a.   ADOT Permit
     b.   Current Copy of Certificate of Liability for $2,000,000

           i.  Include Worker’s Comp
          ii.  Include General Liability
         iii.  Include Auto
         iv.  Name City of Peoria as "Additionally Insured"

3.  FAX to 623-773-7211


CLASS C PERMIT PROCEDURE (Requires Fee for Observation):

1.  Download and complete a Permit Application Form.

2.  Attach the following:

    a.  Map and Survey of Route
    b.  ADOT Permit
    c.  Current Copy of Certificate of Liability for $2,000,000

         i.  Include Worker’s Comp
        ii.  Include General Liability
       iii.  Include Auto
       iv.  Name City of Peoria as "Additionally Insured"

3.  FAX to 623-773-7211

4.  Follow Directions on permit to make arrangements for payment.  Requires a four (4) hour minimum charge.

Customers should allow a minimum of five (5) business days for processing of applications.
