Western Hemisphere Travel Initiative

U.S. Passport Card: 

Customers can now apply for a U.S. Passport Card.  The U.S. Department of State began production of the new passport cards on July 14, 2008.  Applications that were received prior to July 14, 2008 will be processed and distributed between now and early September 2008.

City of Peoria, Passport Acceptance Agency

The City of Peoria Passport Acceptance Agency is located in the City Clerk's Office at Peoria City Hall, 8401 W. Monroe St, Room 150. 

Hours of Operation

Passport Service Hours:  Monday through Friday from 9:00 a.m. - 3:00 p.m.  Passport and Passport Card processing is available to customers on a walk-in basis only.

What Do I Need to Obtain a Passport or Passport Card?

•  Two official passport photos (2"x2")
•  Certified Birth Certificate or
•  Expired Passport which can be used if the applicant is 16 years of age or older or
•  Naturalization Certificate
•  Proof of Identity

Passport Renewals (Application for a U.S. Passport by Mail):

Application forms can be downloaded from the U.S. Department of State's website at http://travel.state.gov/passport/forms/forms_847.html or they may be picked up in person at the City Clerk's Office.  Photo services are available on site.

Fee Information

The City of Peoria will accept checks and money orders only.  No credit card or cash payments will be accepted.  Checks should be endorsed as follows:

City of Peoria = Execution fee (see below)
US Dept of State = Passport fee (see below)
