INSTRUCTIONS
YOU MAY ONLY REPORT INCIDENTS THAT HAVE OCCURRED IN THE CITY OF PEORIA! IF THE INCIDENT YOU ARE REPORTING OCCURRED OUTSIDE OF THE CITY OF PEORIA, PLEASE CONTACT THE APPROPRIATE AGENCY TO MAKE YOUR REPORT.
To begin the report process, click on the words Continue to Report Form at the end of this page. If you are not sure about what information is requested, please refer back to this page for instructions. You can navigate from field to field by hitting the tab key Do not use the enter key on your keyboard until you are ready to send your report.
Step 1: Time and Location of the Crime
Type of Offense
Currently, you may only report the following incident types:
(Pick the category that best describes the incident that you are reporting)
· Theft -The
unlawful taking of the personal property of another. If Person(s)
committing the theft have unlawfully entered an enclosed structure without
permission to enter, do not use the Online Reporting System. Contact the
Peoria Police Department at 623-773-8311.

Criminal Damage (vandalism) Damage done to your property.
· Lost Property - Property that is missing and it is unknown if the
property was taken or lost.
· Bicycle thefts
·
Open Garage Burglaries
·
Traffic Incidents that
occurred over 24 hours prior to report and do not involve injuries,
alcohol/drugs or other criminal acts.

Date and Time of Offense
If you know the exact date of the crime you are reporting, enter that information into the Occurred From field in the following format: mm/dd/yyyy (ex: 03/20/2001). If you are unsure of the exact date, use the Occurred From and Occurred To fields to identify the period of time that the crime may have occurred. Follow this same process to identify the time(s) that the offense occurred by using the hour:minutes format followed by AM or PM (ex: 1:30PM)
Address or Location of Crime
Enter the exact address of the crime if known. If the exact address is not known, enter a specific location using cross streets or any other information in which the exact location can be identified. If the offense occurred at a business, city facility, landmark, etc., please include that information also.
Email
Address of Victim
This is a required field. Enter the Email address
for the victim, whether the victim is a business or a person. If no email
address exists, enter "No Email". The form cannot be submitted
without an entry into this field.
Step 2: If the Victim is a Business
Enter the business name, address, and telephone number if the business is the victim of the crime. If you are reporting a crime involving a business, you must also complete Step 4 of this report form.
Step 3: If the Victim is a Person
Full Name of Victim
Include the full name of the actual victim in this field including middle name or initial if known. If you are reporting this incident on behalf of another person, use that persons information. If the victim of the crime is a business, do not complete these fields Complete Step 2 of this form instead.
Address of Victim
Please list the actual street address of the victim of the crime not the location where the incident occurred.
Home/Alternate Phone Numbers of Victim
Please enter valid phone number(s) of the victim including area codes.
Age and Date of Birth of Victim
Please enter the victims age and date of birth using the mm/dd/yyyy format (ex: 02/01/1962).
Victims Employer, Employers Address, and Work Phone
Please enter the name of the victims employer, the employers address, if
known, and work telephone number, including prefix.
Gender/Race/Ethnicity of Victim
Please select the appropriate boxes that most closely describes the victims gender, race, and ethnicity.
Step 4: If You are Not the Victim
Please complete all fields if you are the party reporting the crime on behalf of a business or other person. If you are the actual victim of the crime, do not complete this category, proceed to Step 3 of the reporting form and complete all fields.
Step 5: Description of the Crime
This is a free form box in which you can type an explanation of what happened. Please be brief and to the point, but provide enough information so that we can tell exactly what happened. An example of a good description would be: I went to the grocery store at approximately 1:30pm. My car was left unattended in the store parking lot for approximately 30 minutes. When I came out, I noticed that the left rear tire had been slashed. A bad example would be: Someone slashed my tire.
Step 6: Lost, Stolen, or Damaged Property
Lost, Stolen, or Damaged
Click the box that applies to the particular item you are reporting.
Description of Item
Provide a description of the item being reported lost, stolen, or damaged. Be sure to include specifics: ex: Red boys 10-speed bicycle with black seat and a Diamondbacks decal on front fender.
Make, Model, and Serial Number
List the make, model, and serial number of all items being reported. If you do not know the information for a particular category, write Unknown in the box.
Est. $ Amt
This is the estimated dollar amount of the lost or stolen item. If the item was damaged, enter the damage amount if known.
Step 7: Vehicle Involvement
How was vehicle Involved?
Not Involved: If no vehicle was involved in the crime you are reporting, leave the default set to Not Involved.
Items Stolen From: If you are reporting a theft that involved an item of property taken from the outside of your vehicle;
i.e., license plate, tools from bed of pick-up, etc, select this option.
Criminal Damage: If your vehicle was damaged, select this option. If both a theft and a criminal damage occurred to your vehicle, select the Items Stolen From option; however, list both the items stolen and the damage to your vehicle in the categories in Step 6.
Driven By Suspect: If you have information on a possible vehicle that was used during the commission of a crime against you, select this option and provide as much details as possible about the vehicle in the fields provided.
Other: If a vehicle was involved in some other manner than those listed above, select this option and fully describe the involvement in the comments section.
Vehicle License Number/State of License Plate
If the vehicle involved was yours, you must provide the License Number and State within the fields provided. You may write Unknown or provide a partial plate number on all others.
Vehicle Make, Model, Type, and Year, VIN, and Color(s)
Provide as many of these details as you can possibly supply. If there is more than one color to the vehicle you are reporting, identify the areas of the vehicle that the each color applies to; ex: White Top and Blue Body.
Comments:
Use this free form box to provide additional details involving the vehicle you are reporting.
Step 8: Send / Reset Form
When you have completed the report and are satisfied with its content, click on the submit button to send the report to the Police Department. Once the report has been submitted, you will receive an acknowledgement message which confirms that your submission was successful. You will receive a return email or telephone call with the case number that we have assigned to your report.
Important
Please complete as many lines of the report as you possible can. This will assist us in accurately screening your report for potential investigation. If you only have partial information
(i.e., part of a license plate number or a persons description), please provide that information within your report.
If you need to report more than one type of crime, please submit separate Report Forms for each one.
IMPORTANT NOTE: Knowingly submitting a false Crime Report is a misdemeanor under Arizona State Law. Dont even THINK about it!
If we cannot contact you to verify the information you have reported, it may not be accepted as an official report.
